I need a copy of a birth certificate. How do I get one?
If the birth occurred after 1909 in one of the five boroughs of New York City (Manhattan, Kings, Queens, the Bronx, or Staten Island), please contact the New York City Department of Health.

If the birth occurred in New York State (outside the five boroughs of New York City), a copy can be obtained from the local Registrar in the city/town where the birth occurred. For birth certificates issued from 1881 to the present, a copy can also be obtained from the New York State Department of Health.
